A typical day as a primary living support worker.
– Develop support plans with customers and coordinate a small team of support workers to work on those plans.
– Conduct risk assessments, support plans, goal setting, and regular reviews.
– Assist clients with activities of daily living, including preparing meals, shopping, budgeting, maintaining rental agreements, and moving to independent accommodation if possible.
– Assist customers with personal care and medication needs as outlined in their support plan.
– Support for attending appointments, participating in hobbies and interests, attending college or work
– Grab a drink, chat, or catch up. There is no need to rush from one customer to the next here. You’ll also have plenty of time to collaborate with colleagues.

– Have, or be willing to work towards, a Level 3 Health and Social Care Certificate;
– We work collaboratively with an eye for detail and are passionate about helping our customers live their best lives.
– Experience in creating person-centered support plans and supporting colleagues in working through the plans.
– Experience coordinating and evaluating customer referrals.
– Take initiative in your work, remain calm under pressure, and have a resilient approach.

Job details
– Even if you can be flexible, you need to meet the needs of your customers. This means that you may need to adjust the start and end times in some cases.
– 24-hour work (including naps)
– Ability to use technology to create and update support plans, complete online learning, and collaborate with colleagues.
– We will need to carry out an extended DBS check, which we will cover the cost of.
